Who: Current 5th, 8th and 11th grade students
Where: CIS and CMS/CHS (MCHD Mobile Unit)
Why: The State of Illinois Physical and Immunization requirements for;
Incoming 6th Grade students
- Physical on State of Illinois Certificate of Child Health Examination form
- Meningococcal Conjugate (MCV4): 1 dose after age 11
- Tdap: 1 dose after age 11
Incoming 9th Grade students
- Physical on State of Illinois Certificate of Child Health Examination form
Incoming 12th grade students
- Meningococcal Conjugate (MCV4): second dose (if first dose of MCV4 administered at age 16 or older; then only one dose required for entry to 12th grade.)
